Sciatica often starts in the back or buttocks and runs down the back or side of the thigh to the calf or foot. Which part of the leg or foot that is involved depends on which part of the Sciatic nerve is affected.

Because the most common cause of Sciatica is a herniated disc at the L4/5 level of the lower back, the pain often goes to the big toe.

When the cause of sciatica is the L 5/S1 disc level of the lumbar spine, Sciatica may go to the outer edge of the foot.

Sciatica may be accompanied by weakness in the leg or foot.

 

Sciatica is nerve pain and the cause should always be determined before an effective treatment regimen can be recommended as sciatica may be caused by a problem other than a herniated disc.

When sciatica is caused by a herniated disc, treatment options include physical therapy, chiropractic and activity modification. Most people will improve without the need for any type of surgery.

For those who do not get better or have a neurologic deficit, surgery is often recommended.

In fact, people with sciatica caused by a herniated disc who only have leg symptoms and no back pain, typically do remarkably well with surgery because once the herniated disc is removed and the nerve is no longer pinched the sciatica resolves quickly – usually, the patient awakes from surgery with immediate relief of the sciatica.

 

Surgery Treatment Options for Sciatica

Sciatica can be treated using traditional open back surgery or minimally invasive spine procedures.

Sciatica that results from spinal stenosis can be treated with an open laminectomy or minimally invasive decompression.

Sciatica caused by a herniated disc can also be treated with traditional open back surgery or minimally invasive spine surgery.

Most people who elect for minimally invasive surgery are educated about their condition and understand the significant risk of scar tissue that results from open back surgery.
